General Purpose: The Infectious Diseases Specialty Clinic Coordinator will serve as the community liaison for the Stride HIV Clinic, as well as facilitate service coordination for Stride’s HIV population.


Essential Duties/Responsibilities:

  • Coordinate care and follow-up services for STRIDE’s HIV and PrEP patients, including scheduling and reminder phone calls. Follow up with patients that no-show to appointments.
  • Schedule financial screening appointments for this patient population.
  • Provide systematic communication with relevant clinic staff to facilitate seamless and reliable service delivery and sustainable expansion to additional sites.
  • Perform data entry as required, including updating city and state agency databases (CAREware & CO HEART) on an ongoing basis.
  • Collect all grant-related data and information per reporting requirements and submit information regularly to designated staff.
  • Outreach to patients that are lost to follow up. Referral to CDPHE Linkage to Care as needed.
  • Work directly with the Sexual Health Program Manager and other program staff to adhere to/and achieve grant specific guidelines and performance measures.
  • Marketing and outreach to community and community partners.
  • Completes all other duties as assigned.

Education and Experience:

  • Required: High School Diploma and 2 years of experience working with people living with HIV or a Bachelor’s Degree.
  • Required: Active BLS certification.
  • Background in administrative and clerical duties.
  • Experience working with people living with HIV preferred.

Knowledge, Skills and Abilities:

  • Possess sensitivity to and understanding of the varied needs of people living with HIV.
  • Experience providing outreach and intervention programs.
  • Experience working with patients with multiple issues.
  • Ability to interact positively and build rapport with patients, co-workers and external contacts.
  • Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of STRIDE’s patient population. · Ability to handle sensitive information ethically and responsibly.
  • Ability to protect the confidentiality of patient, employee and business information.
  • Strong organizational and time management skills

Work Schedule

Monday-Friday, 8 hours/day

Hybrid: 4 days in clinic, 1 day working from home


We offer a competitive hourly range of $20.67 - $24.03, depending on your experience.

This range reflects STRIDE’s good faith estimate of potential compensation at the time of posting. The final salary for the selected candidate will be determined based on several factors, including experience, education, budget, internal equity, specialty, and training.

Material and equipment directly used: Computer, printer, multi-line telephone systems, fax machine, copy machine, calculator, Electronic Health Records (EHR) system and all Microsoft Office products including Outlook, Excel and the internet.


Working environment/physical activities: Office environment within a clinical setting. Enters data into computer programs via computer, mouse and keyboard. Moves about the office environment and occasionally to other. Moves/transports objects up to 25 lbs. occasionally. Communicates information to others. Discerns/analyzes information from others to assist in decision making.
