Essential Job Functions

  1. Confers with residents, families and the campus staff throughout the admission process for the campus.
    1. Handles initial walk-in inquiry and completes resident and/or family tour by coordinating with campus staff in desired area of residency.
    2. Interprets financial/administrative policies explaining accurately and completely the services and fees of Lutheran Life Villages, and the roles of various governmental and private insurance programs.
    3. Coordinates all assessments with the Director of Nursing for residents that are not currently in the hospital.
    4. Communicates hospital referrals to the Community Liaison for follow up by the Community Liaison.
    5. Communicates to resident and families both our commitment to quality care and client's payment responsibilities for services rendered.
    6. Obtains financial information required to complete the Admission Application.
    7. Complete required Admission Packets with Resident and Resident Representative, if applicable
  1. Reviews the admission process and related forms for necessary revisions.
  2. Updates/revises/refines admission records and procedures.
  3. Arranges transportation for residents from the hospital as new admissions.
  4. Completes benefits verifications in Point Click Care.
  5. Ensures High Performing Network Providers, Acute Care, and Long-Term Care providers are notified of incoming admission, pertinent information, and updates.
  6. Demonstrates a cooperative working relationship with community agencies, hospitals, nursing facilities, and other referral sources to maintain smooth admission procedures.
  7. Works closely with the neighborhood coordinator during the admissions process to find the appropriate room during the resident's stay.
  8. Ensures Medicaid approved residents are located in Medicaid-licensed bed.
  9. Assist residents, responsible parties and families with financial and/or admissions questions either by personal visits or by phone.
  10. Completes Census updates including:
    1. Daily email notification
    2. Census spreadsheet, which will be submitted to the Business Office
    3. PointClickCare ADT submissions and other data entry into the system
  11. Monitors and updates the Medicaid and Long-Term Care list.
  12. Completes campus tours and inquiry calls.
  13. Updates daily the Aline system with data entry related to admission leads and admissions in process, ensuring pertinent information is included in the charting.
  14. Participate in Employee Appreciation Team and assist Employee Appreciation Team as needed.
  15. Access EPIC EMAR system in accordance with High Performing Network expectations and communications.
    1. Complete admission process in this system as directed.
  16. Performs any other duties as assigned.
