The Healthcare Surveyor II/Community Programs Healthcare Surveyor will:
- Conduct compliance surveys of Home and Community Based Service Waiver programs providing services to individuals with developmental / intellectual disabilities, mental illness and other disabling conditions;
- Ensure compliance with state and federal requirements, analyze data and write formal reports of findings;
- Conduct on-site interviews, record reviews and observations.
How does it get done?
The Healthcare Surveyor II/Community Programs Healthcare Surveyor is responsible for:
- Having knowledge of and ability to learn applicable federal and state standards and regulations affecting programs and agencies involved in providing waiver services;
- Participating as a Team Lead or Team Member in compliance surveys as assigned, including Statewide travel throughout New Mexico;
- Preparing for compliance surveys, participating in compliance surveys, and completing formal reports of findings, This includes having the ability to coordinate and lead a compliance survey, communicate logistical information regarding the survey to all involved team members, and provide leadership to the survey team by providing clear and concise information; .
- Possessing skills in gathering, interpreting and analyzing data and making informed decisions to determine compliance status of the providers based on standards and regulations;
- Successfully complete all required HCA, DHI and DDSD trainings, including QMB core surveying competency training;
- Providing excellent customer service to internal and external customers of QMB and DHI.
Who are the customers?
- Quality Management Bureau (QMB) of the Division of Health Improvement (DHI) works in close coordination with other divisions withing NM Health Care Authority and with other State agencies to ensure accountability and compliance with state and federal, program, contractual and quality standards;
- Individuals receiving DDW, Mi Via, Med Frag Waiver and Supports Waiver services;
- Families/Guardians of Individuals receiving DDW, Mi Via, Med Frag Waiver and Supports Waiver services;
- Provider agencies;
- The community at large.

Minimum Qualification
Associate's Degree in from an accredited college or university in Nursing, Architecture, Social Work, Sociology, Psychology, Rehabilitation and/or Criminal Justice and two (2) years of experience in community based healthcare services and case management. Employment Requirements
Must possess and maintain a valid New Mexico Driver's License. Must Possess and maintain a current Defensive driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.
Working Conditions
Work is performed primarily in an office setting, nevertheless, there is extensive travel throughout New Mexico. Worked is performed at community provider agencies and out in the community during home and community site visits. Position requires extensive use of a computer and telephone, including; extended periods of sitting; talking and frequent standing; requires the ability to lift and carry over 25 lbs; requires.
