Resident Care Coordinator shall provide for the needs of the residents, the resident care associates, and the resident care department. The RCC shall assist with the overall clinical/wellness operation. The RCC, in conjunction with the Resident Care Director (RCD), shall optimize the efficiency and the effectiveness of the resident care department.

Essential Functions, Duties and Responsibilities:

  • Participate in and complete the communitys assigned in-service training requirements, whether they be in person, in writing or online.
  • Work with the team, to monitor for changes in resident needs, preferences and health status. Report/document any noticed changes at once to the Resident Care Director, RN or designee.
  • Prepare the Resident Care staff work schedule, to include RCAs, Medication Aides, Nurses, etc. The work schedule shall be posted within the community minimally 2 months in advance. Manage all Resident Care staff attendance.
  • Daily review of time clock punch detail, hours per resident day, overtime hours and agency use. Daily report to be given to the RCD/ED. Based on review of time clock detail, complete missed punch forms and submit to Business Office Director for accurate time-keeping.
  • Order all clinical and incontinence supplies.
  • Provide orientation and training to new Resident Care team members on the use of the electronic health record and medication administration record.
  • Monitor and review Resident Care Associate training logs and assist with creation of monthly training calendar. Report monthly to RCD, BOD, and ED regarding RCA training compliance.
  • Document both scheduled and unscheduled services that are provided, including incidents, behaviors, and out of the ordinary events, including a residents refusal of services. Input information in to the community computer tracking system regarding services provided to residents.
  • May conduct or facilitate programming, including therapeutic groups and individual interaction, that offers cognitive stimulation, socialization, physical activity and spiritual connection.

Qualifications

Job Qualifications:

  • The job description qualifications (certifications, experience, training), job functions, including limitations may vary with individual states.
  • Knowledge, Skills and Abilities:
    • Ability to use computer for documenting services provided.
    • Shall be fluent in the ability to communicate in the English language, both oral and written.
    • Organizational skills and proficiency in Microsoft office is essential.
    • Demonstrates computer literacy and familiarity with office productivity software (e.g. Microsoft Word, Excel), and has the ability to learn and use new software platforms (e.g. Yardi, OnShift, etc.),
  • Education: Must have completed the course for certified nursing assistant or have equivalent training or other related experience.
  • Experience:
    • Prior experience providing direct care or the equivalent in training is preferred.
    • Experience in caring for seniors with Alzheimers disease or other forms of dementia is preferred.
    • Experience with nursing/wellness operations for senior housing is preferred.
  • Certificates/Licenses: Current license/certification preferred as required by State Regulation and provide copies of the documentation to the Resident Care Director or designee.
  • Physical Demands:
    • Finger and hand dexterity to handle and manipulate instruments and equipment.
    • Visual and auditory acuity.
    • Must be able to lift a minimum of fifteen (15) lbs. and be able to carry objects.
    • Must be able to push and pull carts, and continuously walk, sit and stand.
    • Must be able to turn, crouch, kneel, stoop, bend below waist, and reach above shoulder height as tasks dictate.
    • Must use good body mechanics. Stamina to make visits to residents throughout the community.
    • Occasional pushing or pulling, assisting of residents weighing (~150 lbs.).
    • Have the ability to occasionally push an occupied wheelchair for a distance of up to 200 feet.
    • Must project a positive attitude towards the service provided.
    • Associate may be subject to potential exposure to blood or other potentially infectious materials, and to handling hazardous chemicals (either Category 1 or 2).
