We provide housing resources such as rental subsidies, housing navigation services, and permanent supportive housing (PSH). A DAY IN THE LIFE OF A SUPPORTIVE HOUSING CASE MANAGER

In this case management position, you provide housing and social services to individuals who struggle to maintain their residences. Following the "housing first" model, you focus on helping individuals maintain a stable place to live while addressing other concerns. Because housing is essential for individuals to be able to work towards other goals, that is always your priority.

You also assist clients with addressing other problems such as benefits access, symptom management, educational goals, vocational goals, and more. Conscientiously, you monitor and evaluate each client's progress towards specific goals around housing, employment, and harm reduction. Additionally, you help to provide skills for behavioral health, medication management, financial management, and activities of daily living. QUALIFICATIONS FOR A SUPPORTIVE HOUSING CASE MANAGER

This case management position will meet one or more of the following requirements:

  • Bachelor's degree in mental health, social services, or a related field, plus one year of experience relating to or assisting individuals with mental illness
  • Three years of relevant work, education, training, and/or experience
  • Certification as a qualified mental health associate through the Mental Health and Addiction Certification Board of Oregon or the ability to meet the criteria to be eligible for certification after one year
  • Lived experience plus a combination of two years of employment, education, or volunteer work
  • Registration on the Oregon Traditional Health Workers' Registry as a PSS, CHW, or PWSS

Additionally, candidates are required to fulfill requirements set by the Laurel Hill Center, including:

  • Valid Oregon driver's license
  • Willingness to undergo a background check and drug screening upon hire
  • Full vaccination against COVID-19

WORK SCHEDULE FOR A SUPPORTIVE HOUSING CASE MANAGER

This full-time behavioral health position typically works four 10-hour days, either Sunday - Wednesday or Wednesday - Saturday. Hours are between 8:00 am - 8:00 pm.
